Emergency Summary Care Record

NHS Summary Care Records were introduced to improve the safety and quality of patient care.  They are electronic records which give healthcare staff faster, easier access to important information about you to help provide you with safe treatment when you need care in an emergency or when your general practice is closed.

The London Lane Clinic uploads summary care records.  They contain

  1. Information about medication you are taking
  2. Allergies you suffer from, and
  3. Any bad reactions to medicines that you have had

as long as we have a record of this.  Your Summary Care Record also includes your name, address, date of birth and NHS number in order to identify you correctly.  It is not the whole medical record.  It is used by clinical staff at the local out of hours service for your care.

You can opt not to have a Summary Care Record if you let us know by giving us a completed, signed and dated opt-out form.  We will then code your record so that it cannot be uploaded.
