This is an NHS general practice. We have an open list and welcome requests for registration from patients living in our practice catchment area. Our catchment area covers most of the BR1 postcode area along with some of BR2 and BR7 extending from Scotts Lane/Pickhurst Lane in the west to Lower Camden/Blackbrook Lane in the east, and Downham Way in the north to Crown Lane/Southborough Lane in the south. Bus routes 208, 320, 126 and 261 all pass close to London Lane. Please click on the practice boundary link and enter your postcode to see if you live in our catchment area.
You can register by:
- complete the registration form by clicking on the following link http://www.nhs.uk/servicedirectories/Documents/GMS1.pdf then either emailing this to us via email@example.com with a copy of proof of ID and address, or by printing off the form and presenting this at the surgery with your ID and poroof of residency. *Please note that every part of the form needs to be completed as all this information is needed to enable us to process the application.
- If you cannot do this electronically, forms are available for collection at the surgery.
You will need to bring separate proof of identity, and of residency (eg a current utility bill, bank statement or council tax letter). You must provide your NHS number, as without this we cannot register you.
All patients have a “named GP”. Your named GP will be responsible for co-ordinating aspects of your care and you are encouraged to see this GP whenever possible. However, you will still be free to see any doctor at the practice if they are available.
You may register at the practice on a weekday between 2pm and 6pm or alternatively, contact us for Saturday opening times.
New patients will be invited to attend a short new patient health check with a Health Care Assistant or Practice Nurse. This health check will help us identify any current problems, record basic medical data, and give new patients an opportunity to ask relevant questions.
If you have a condition which affects your ability to communicate eg hearing loss, problems with vision, learning disability, aphasia, autism or mental health problems, please make sure Reception know, so that we can take steps to ensure that we communicate with you in a way that you are best able to understand.
Patients are registered without discrimination.
If you have a visitor staying with you who needs to see a doctor, they can be registered at the London Lane Clinic as a temporary patient if they are eligible for NHS care. The temporary registration period can last up to 15 days or up to 3 months depending on how long the patient is staying at the temporary address. Please contact Reception for details.
